GLP-1
Coming off Ozempic or Wegovy without the rebound

The short answer
Most of the rebound after stopping a GLP-1 comes from appetite returning to a body that lost muscle and never built new habits. In the STEP 1 extension, people regained about two-thirds of their loss within a year of stopping. Keep lifting two to three times a week, keep protein high, decide on a weight range that triggers action, and plan any change with your prescriber.
The medication worked. That's the good news, and it's real. The uncomfortable part is what the research shows happens after people stop, so let's look at it straight and then talk about what you can actually control.
Quick note before anything else: whether and how you come off a GLP-1 is a decision for you and your prescriber. Everything in this post is about the training and food side, which is my lane.
What happens when you stop Ozempic or Wegovy?
Appetite comes back, and for a lot of people so does much of the weight. The best data is the STEP 1 trial extension, which followed 327 people for a year after they stopped semaglutide (Diabetes, Obesity and Metabolism, 2022).
| During treatment (68 weeks) | One year after stopping | |
|---|---|---|
| Average weight change from start | -17.3% | -5.6% |
| Share of the loss regained | ~two-thirds |
So on average, people ended a year later still 5.6% below where they started, which isn't nothing. But two-thirds of the hard-won loss came back. On a 200-pound starting weight that's ~35 pounds lost, then ~23 regained, leaving ~11 down.
One detail most summaries skip: in that trial, the lifestyle program was stopped at the same time as the drug. The rebound happened when both supports disappeared at once. That's the part you can change.
Why does the weight come back after stopping a GLP-1?
Two things collide: appetite returns to normal, and the body it returns to may be carrying less muscle than before. Less muscle means fewer calories burned at rest, so eating the way you used to puts weight on faster than it used to.
In the STEP 1 body-composition substudy, lean mass fell 9.7% on semaglutide alongside a 19.3% drop in fat (Journal of the Endocrine Society, 2021). Some of that lean mass is water and organ tissue, not all muscle. Still, it's the reason I harp on lifting during treatment. Muscle you keep on the way down is muscle working for you on the way off.
How do you keep the weight off after stopping Ozempic?
Build the habits while the medication still makes them easy, keep lifting, keep protein high, and decide in advance what you'll do if the scale creeps. That's the whole plan.
- Start 8 to 12 weeks before any change. Habits that take hold while appetite is low are far easier to keep when it returns.
- Keep strength training two to three times a week. A 2025 joint advisory from four obesity and nutrition societies recommends GLP-1 therapy be paired with strength training at least three times a week plus 150 minutes of moderate aerobic activity (Obesity, 2025). That doesn't stop when the prescription does.
- Keep protein at every meal. The same advisory notes 80 to 120 grams a day as an absolute target during weight loss. In maintenance it still anchors meals and helps with fullness.
- Eat on a schedule. Regular meals beat "I'll eat when I'm hungry" once hunger comes back loud.
- Pick a trigger range. Decide now: if I'm 4 pounds above my end weight for two weeks in a row, I tighten up. Deciding in advance beats deciding while you're frustrated.
What should your training look like after stopping?
The same as during treatment, and maybe a little more. Keep the progressive lifting, keep the walking, and keep measuring body composition.
The CDC's floor for every adult is 2 days of muscle-strengthening and 150 minutes of moderate activity a week (CDC). Coming off a GLP-1 is a bad time to hover at the floor. The training plan itself is laid out in keeping your muscle on Ozempic, and it carries over unchanged.
How do you handle hunger when it comes back?
Expect it, plan for it, and don't read it as failure. Your appetite returning isn't a character flaw. The medication was suppressing a normal signal, and now the signal is back.
- Front-load protein early in the day, when the habit is easiest to keep
- Keep the same meal times you built on the medication
- Keep high-volume, filling foods around: vegetables, fruit, soups, lean proteins
- Watch liquid calories and evening grazing, which are where most quiet regain happens
The food side is broken down with a sample day in what to eat on Ozempic to keep your muscle. The same structure works in maintenance with slightly bigger portions.
Should you track your weight after stopping?
Yes, weekly, and body composition every month or two. Weekly weigh-ins catch drift before it becomes 15 pounds, and scans tell you whether you're keeping the muscle.
Daily weigh-ins drive a lot of people crazy because water alone can swing the number a couple of pounds overnight. Weekly averages are calmer and more honest.
Do you need a coach for this part?
You don't strictly need one. But this transition is where accountability matters most, because the medication was doing a lot of the work quietly and now something has to replace it.
One pitch, labeled as such. If you're in Flower Mound and coming off or planning to come off a GLP-1, here's how GLP-1 support works at my studio. The free 6-Week Challenge starts with a body-composition scan, so you know exactly what you're working with before the transition starts. If you're over 50, strength training after 50 when you've never lifted is the gentlest place to begin.
Common questions
Will I gain the weight back after stopping Ozempic?
Many people regain a large share. In the STEP 1 trial extension, people who stopped semaglutide regained about two-thirds of their prior weight loss within a year, though they stayed about 5.6% below their starting weight on average.
How do I stop Ozempic without gaining weight?
Talk to your prescriber about the plan, and start the habits months before the last dose: strength training two to three times a week, a daily protein target, regular meals, and a set weight range that tells you when to tighten up.
Why do people regain weight after GLP-1 medications?
Appetite comes back, often stronger than it felt on the medication, while the body may be carrying less muscle than before. Less muscle means fewer calories burned at rest, so old eating habits lead to regain faster.
Should I taper off Ozempic or stop all at once?
That is a medical decision for your prescriber, not your trainer. What a coach can do is make sure your training, protein and eating structure are solid before and after whatever plan you and your doctor choose.
Does exercise prevent weight regain after Wegovy?
Strength training protects muscle, and muscle protects your resting metabolism, which is one of the biggest levers you control. It works best combined with protein, regular meals and tracking, rather than as a stand-alone fix.
